Mountain Shadows Community Homes - San Diego

Nestled in the foothills of San Diego’s North County, Mountain Shadows Community Homes – San Diego is comprised of nineteen individually licensed homes for the developmentally disabled; Intermediate Care Facility for the Developmentally Disabled; and currently 16 Habilitative (ICF DD/H) and three Nursing (ICF/ DD-N), though we are in the process of converting a Habilitative ICF DD H to a third Nursing (ICF/ DD-N) residence. Sixteen homes are situated on five sprawling acres located in Escondido, and the remaining three community-based homes are in the heart of San Marcos. Each home is designed to foster a family environment to encourage the growth and independence of all our residents.

Mountain Shadows Community Homes strives to provide the best possible environment for our residents to achieve maximum independence and improved self-esteem through superior habilitation, training, and support systems. Mountain Shadows Community Homes – Riverside

Mountain Shadows Community Homes – Riverside has eight residential homes in Riverside County. Each home is licensed by the California Department of Health Services as an Intermediate Care Facility for the Developmentally Disabled – Nursing (ICF/ DD-N). We provide services for infants to young adults. At Mountain Shadows Community Homes, we strive to provide a loving and nurturing environment that fosters growth and independence to the greatest extent possible for each child. Every child’s program is individualized to best meet all medical, developmental, and social needs.

The mission of MSCH is to promote optimal health, development, and independence for medically fragile individuals with or at risk for developmental delays in homes within the community through exceptional nursing and supportive services by a nurturing team of professionals.

Mountain Shadows Community Homes provides active treatment for each consumer. Active treatment is an organized effort to maximize each child’s potential.

Staff members are trained to provide a quality learning environment directed at developing skills and preventing possible regression. Individual specific service plans include:

1) Physical Therapy
2) Occupational Therapy
3) Speech Therapy
4) Recreational Therapy
5) Community Outreach
6) Nursing Care Plans

We provide exceptional nursing services to ensure all children receive excellent care by educated staff members. Our highly trained staff, consisting of Direct Care Staff, LVNs, RNs, and on-call physicians, provides tube feedings, ostomy care, oxygen therapy, apnea treatment, daily nursing assessments, and medication administration.
