Mountain Shadows Community Homes San Diego & Riverside

MSCH offers 24/7 care in a residential program setting specifically designed to cater to individuals with intellectual disabilities. Specifically, we operate 16 Intermediate Care Facilities for the Developmentally Disabled – Habilitative (ICF/DD-H) and 9 Intermediate Care Facilities for the Developmentally Disabled – Nursing (ICF/DD-N).

Our programs are tailored to provide active programming and support in a residential setting. We are committed to serving individuals of all ages, from children to seniors.

Our dedicated team of professionals strives to create a nurturing and inclusive environment where every resident can thrive. With personalized care plans, we focus on enhancing life skills, promoting social engagement, and encouraging independence. Our licensed homes include comfortable living spaces, recreational back yard areas, and access to community resources, ensuring that each individual feels at home. We believe in fostering a sense of belonging and empowerment, helping our residents to lead fulfilling lives while building meaningful connections with peers and staff alike.

We offer the following services:

  • Physician and nursing services, physical therapy, occupational therapy, speech therapy, behavioral management, nutritional services, meal assistance, medication administration and hygiene assistance.
  • Advanced medical and nursing services in our ICF/DD-N homes, including assistance with BiPap, CPAP apnea monitoring, colostomy and ileostomy care, medication and feeding administration via gastrostomy and/or jejunostomy tubes, intermittent catheterization, nebulizer treatments, oxygen administration, oral suctioning, vesicostomy care and stage one and two wound care.
  • Connecting residents with their selected adult day programs fostering community integration, such as vocational training, assistance in procuring jobs, and/or school and educational transition programs through the local school district.
  • A fleet of custom-designed, accessible buses and vans to transport individuals safely to medical and dental appointments, Day Programs, and community outings to shopping centers, restaurants, parks, and theaters.
  • A robust recreational activity program to encourage each individual to enjoy life to the fullest, with trips to the Safari Park, Balboa Park, Petco Park, Living Desert Zoo, Disneyland, Aquarium of the Pacific, Sea World,  San Diego Zoo, skiing in the San Gabriel Mountains, adaptive aquatic camps and more.

MSCH-San Diego

In San Diego County, MSCH manages a campus facility that covers roughly 5.4 acres in Escondido. This campus features sixteen individually licensed intermediate care homes, which together can accommodate up to 105 residents. Furthermore, we also operate three community-based homes in San Marcos, with each home providing residence for six individuals.

The campus in Escondido is designed to cater to a variety of needs, with amenities that promote well-being and comfort. Lush gardens and paths offer serene spaces for relaxation and reflection, while recreational areas encourage social interaction and physical activity.

In San Marcos, the community-based homes are integrated into the neighborhood, allowing residents to maintain an active presence in the local community. These homes emphasize independence and personal growth, with staff providing assistance as needed to empower individuals in their daily lives.

Collectively, these facilities reflect MSCH's commitment to enhancing the quality of life for those they serve, ensuring that each resident feels valued and supported in a safe and nurturing environment.

MSCH-Riverside

In Riverside, MSCH offers its services to the Jurupa Valley community, managing six distinct licensed intermediate care nursing (ICF/DD-N) homes. Our main goal is to enhance the health, development, and independence of medically fragile individuals who have or are at risk for developmental disabilities. We accomplish this through outstanding nursing and supportive services delivered by a compassionate and dedicated team of professionals.
